Epidemics and control of hepatitis B in ChinaChinese Full Text

Abstract: Hepatitis B virus(HBV)infection is a global public health issue and poses a greater disease burden in China particularly.Since 1992,China introduced the hepatitis B vaccine into routine immunization management,through the integrated mechanism to encourage the hospital delivery rate,which greatly contributed to improving the timely birth dose and three dose coverage of hepatitis B vaccination.As a result,it reduced mother-to-child transmission of HBV infections.Furthermore,China has been strengthening blood screening and disease diagnosis,detection and disinfection of standard management for nosocomial infection in hospitals,effectively controlled iatrogenic HBV infection,and new HBV infection rate dropped significantly.Thanks to the comprehensive strategies,China has reached the goal set by the Western Pacific Region Office of World Health Organization(WHO)that by 2017 the HBsAg prevalence gets to less than 1% among children under 5 years old.In addition,through the implementation of health promotion,the public awareness of the prevention and control of hepatitis B has been improved dramatically,and the discrimination for school admission and job recruitment have been reduced by government′s legislation.However,there are still quite a lot of number of mother to child transmission occurring in newborns;certain adult populations are now part of the high-risk groups infected with HBV,community HBsAg positive awareness is low,the standardized follow-up and treatment still face challenges.In summary,the HBV infection in China still remains a big issue,brings the high disease burden.To fundamentally control the HBV infection,China has to maintain vaccination policy for infants as a priority,make the catch-up vaccination to children as the routine immunization supplement and consider routinely conducting the high-risk adult vaccination.Furthermore,it is urgent to strengthen the nosocomial infection control,raise people′s awareness,increase the screening,diagnosis and treatment of HBV infections.China should realize these key strategies to reach the WHO target for 2030.
