CO2 Laser Surgery in Cancer of Upper Aerodigestive Tract

摘要:Since the introduction of the carbon dioxide(CO2) laser into clinical otolaryngological procedures,transoral laser surgery(TOLS) for malignant tumors in the upper aerodigestive tract has been gaining increasing importance.Transoral microscope resection with CO2 laser is a safe and unquestionable treatment for early tumours of the upper aerodigestive tract.However,radiotherapy,usually applied for early stage tumors,may have several complications that adversely affect quality of life.By means of TOLS,in early stages of the disease,we achieve local control,thus,avoiding the adverse effects of radiotherapy and improving both voice and swallowing quality.Transoral laser surgery for advanced cancer treatment,in well-selected patients,also provides results that can be comparable to other oncological treatments.Treatment with TOLS may be applied with positive results after chemoradiation failure or recurrence after failure.Nowadays,laser surgery plays a leading role in minimally invasive techniques in OtolaryngologyHead and Neck Surgery,particularly in the treatment of the upper aerodigestive tract malignancies,including more advanced cases,where the aim is to preserve the organ reducing patient morbidity and providing faster recovery.W.Steiner enunciated that the principle is to "minimize the surgical morbidity without compromising oncological principles".
