Peripheral T-cell Lymphoma of Tongue-Report of a Rare Case and Review of Literature

摘要：Extra nodal lymphomas account for 20-30%of non-Hodgkin Lymphoma（NHL）.Primary nonHodgkin lymphomas（NHL） of the oral region are rare accounting for 3-5%of all malignant lesions of which Diffuse Large B-cell lymphoma（DLBCL） is the most common type.Most patients are asymptomatic and rest of them present with mucosal ulceration.Oral NHLs may mimic benign orodental conditions and hence are easily misdiagnosed.Understanding the biological behavior and therapeutic options of oral cavity lymphomas is difficult due to paucity of cases.Little is known about the etiological factors of primary lymphomas of the oral cavity.Apart from age,there are several risk factors associated with NHL like Primary or acquired immunodeficiency,Autoimmune disease（Sjogren’s syndrome,Celiac disease,treatment with immunosuppressant drugs）,Infective agents like Herpetic virus,Hepatitis C Virus,Helicobacter pylori,exposure to chemical agents and Hereditary factors.A proper clinical evaluation,histopathologic and immunohistochemical evaluation will help in the diagnosis and management of the cases.Of these Peripheral T-cell lymphomas（PTCL） are extremely rare with only a few cases reported in literature.We describe a case of 50 yr old lady who presented with an indurated lesion on the tongue.Local examination revealed a 2*2 cm sub mucosal indurated lesion involving lateral margin of left side of dorsum of tongue.MRI showed a well defined T2 W hyper intense lesion in left half of anterior portion of tongue measuring3.2*2.5*1.7 cms.PTCL was diagnosed after immunohistochemistry and T-cell receptor gene rearrangement.Though PTCL of oral cavity are extremely rare,the possibility should always be considered in the differential diagnosis of T-lymphoid proliferations affecting this area.In this lecture,review of literature,work up strategy and various aspects related to differential diagnosis and prognosis of oral cavity lymphomas will be discussed.
