摘要:Objective: To determine if pupil near response can act as a new index of behavioral assessment to evaluate consciousness in patients with disorders of consciousness(DOC).Methods: Pupil near response(PNR) was evaluated in 20 patients who had been diagnosed as UWS by CRS-R.Their recovery of consciousness was also assessed by CRS-R 3 months after to see if the PNR has prognostic value to them. Results: All 4 patients with PNR had favorable prognosis, whereas only 31.25%(5/16) patients without PNR had the same.Furthermore, all PNR+ patients showed conscious visual behaviors in the follow up,whereas only 12.5% PNR-patients did the same. Conclusions: The patients who showed pupil near response all had favorable prognosis in the follow up,suggesting it has relationship with consciousness. Moreover, for it occurred with the absence of visual conscious behaviors of CRS-R in the CRS-R assessment before the PNR test and the objective index(ratio of pupillary constriction) we chose to represent its presence,it is the solution to the subjectivity and misdiagnosis of current evaluation of consciousness in DOC patients.
