摘要:Thalamus has been shown to play a major role in transmitting and regulating perception of sensation related to pain. Deep brain stimulation(DBS) of sensory thalamus has been suggested to ameliorating the chronic neuropathic pain in patients. However, the neuralmechanisms underlying the role of thalamus in neuropathic pain and the relationship between thalamus function and pain treatment is unclear. The present study aims to evaluate the relationship between thalamic neural oscillations and pain relief by DBS for neuropathic pain, by recording the local field potentials(LFPs) in sensory thalamus. The thalamic LFPs were recorded from thirteen patients at rest. All patients were instructed to rate their pain on a visual analog scale(VAS; 0-10, 0 = no pain, 10 = worst pain ever experienced) both before and after deep brain stimulation. The rhythming of thalamic LFPs was characterised with spectral analysis. The balancing and coupling between thalamic oscillations was quantified by the amplitude ratio and cross correlation between different frequency spectral components. The relationship between these characteristics and pain relief was evaluated by nonlinear regression analysis. The results showed that prominent spectral peaks between 2 Hz and 40 Hz were observed more often in the alpha band(8~12 Hz, 8/13 patients) than the theta band(4~8 Hz, 6/13 patients) or the beta band(12~30 Hz, 5/13 patients). The power at theta, alpha, high beta and 50~60 Hz was significantly correlated with VAS reduction(R=-0.64, 0.49, 0.76, 0.52, p<0.005, respectively). The power ratio of alpha to theta, high beta to delta, high beta to low beta, 50~76 Hz to 40~50 Hz was correlated with VAS reduction(R=0.71, 0.72, 0.65, 0.69, p<0.005, respectively).The power coupling between the paired frequency bands(12~20 Hz, 20~30 Hz),(50~60 Hz, 80~90 Hz),(60~70 Hz, 70~76 Hz) and(2~8 Hz, 60~76 Hz) was correlated with VAS reduction as well(R=-0.65,-0.65,-0.83, 0.68, p<0.005, respectively). These results suggested that the thalamic neural oscillations exhibited complex behaviour in patients with neuropathic pain. The integration of rhythming, balancing and coupling behaviours was related to the pain relief by DBS and it provides objective and quantitative biomarkers for pain relief.
